How do I complete a SystmConnect?
- Go to www.theleighsurgery.nhs.uk
- Look for the banner that says ‘SystmConnect is now available to submit medical queries’
- You can log in using SystmOnline or NHS login. Note: Registration is NOT required.
- Logging in will only bring in your name and patient details.
What happens after I submit my form?
- Administrative requests will be handled by our admin team as usual.
- Clinical requests will be handled, prioritising your clinical need (red, amber, green).
- You’ll receive a text message outlining the next steps, which may include a scheduled appointment or a link to book an appointment at your convenience.
- Your query may be resolved without an appointment, or you may receive advice on self-care or alternative services.
Why is The Leigh Surgery making this change?
- This system has been shown to improve access to appointments and prioritise patient care, ensuring the most appropriate healthcare professional sees you.
What if I am unable to complete a SystmConnect online?
- Our friendly team at The Leigh Surgery are happy to support and guide you through the new process.
Will I be able to see the doctor I usually see?
- Yes, you can add your preferred doctor in the SystmConnect ‘further information’ section. Alternatively, choose your doctor when using the self-book link.
Who is going to triage my SystmConnect?
A member of the team will review your request. The daily triage team consists of:
- Doctor, Practice Nurse, Administrator. They work together to ensure appropriate triage.
How will my appointment be booked after triage?
- The default method is via text message.
- You will be offered a link to book an appointment (amber & green) at a convenient time for you.
- Patients deemed to require urgent (red) assessment will be offered an appointment on the same day.
Note: Unless otherwise specified, all clinical triaged requests will be face-to-face appointments.
What if my problem is urgent?
- Please submit an online form as early as possible so our clinical team can address your needs.
Can I request a prescription using the online form?
- Yes, but for repeat prescription requests, the NHS app or SystmOne Online are the simplest way to submit a repeat prescription request.
What if I don’t have internet access?
- We have a tablet with free Wi-Fi in reception, and you are welcome to use our Wi-Fi with your own device.
What if a patient is housebound, elderly, or unable to use online systems?
- We will identify and support vulnerable patients who cannot use the online form. We encourage family members or carers to use the form where possible.
What if I do not want to disclose why I am contacting the surgery?
We understand sensitive situations; however, providing some information helps us:
- Get you the right help with the correct clinician.
- Ensure appropriate and timely care.
This could include details about: New or ongoing health issues, whether your condition is changing and any specific concerns you have.
