Register as a new patient

Online registration

Our practice now offers online registration. It takes around 15 minutes to complete a registration. You need to complete the process in one session.

If you are registering more than one person make sure to give yourself enough time. Your chosen GP surgery should register you within 2 weeks of receiving your details.

Alternatively, to register as a new patient with our practice, please visit our practice and complete the following forms:

• Registration form
• New patient questionnaire

You will also need to provide:

• Photographic ID
• Proof of address

If you do not have these documents, please inform our staff when returning the completed forms so they can assist with your registration.

If you’re homeless, you can use a temporary address such as a friend’s house, shelter, or the GP surgery address.

Getting assistance

If you need help completing the registration forms, please call our surgery. You can also seek assistance from:

• Local organisations supporting the homeless
• Citizens Advice
• Your local Healthwatch

Registration refusals

A GP surgery can refuse registration if:

• They are not accepting new patients
• You live outside their practice boundary area
• You were previously removed from their patient list
• It’s too far from your home for home visits

Changing GP practices

You can switch to a new GP surgery if:

• You have moved
• You had problems with your current practice
• You were removed from the patient list

Notify your old GP when changing address or leaving their area.

Special circumstances

Under the Mental Capacity Act, if a person over 16 cannot make decisions about their care, registration can be done by:

• A relative
• The main carer
• A power of attorney
• A court-appointed representative

Temporary residents

If you require treatment while away from home or are not currently registered with a GP, you can register as a temporary resident for up to 3 months at any surgery.
